Before your first visit

Application for Admission
Recent report cards (academic and socialization records)
A letter of recommendation from a medical or educational professional
A current psychoeducational evaluation (recommended, if available)

Before your student starts

Copy of birth certificate or passport
Copy of Social Security card
Health/immunization records (Florida DH Form 680)
School entry health form (Florida DH Form 3040)
Transfer information from your student's previous school
